Care-at-home program for heart failure patients
A healthcare client engaged Deloitte to develop a scalable virtual health platform that significantly improves the patient experience outside the walls of the hospital to reduce preventable and costly readmissions. We leveraged Deloitte’s “MyPath for Connected Care” platform and built out two iOS apps, one for patients and one for care teams, for the initial use case of congestive heart failure (CHF).

My Role & Approach Summary:
I led overall project management and launch strategy (including a human-centered design workstream) across a ~30-person Hybrid Agile team. The engagement was structured as three main phases:
Discovery and initial design | Conducted research to define the moments that matter along the patient journey for patients and clinicians. Aligned on the minimum viable product (MVP).
Final design and MVP configuration | Iterated on the end-to-end user experience and validated UI/UX through additional user interviews and usability testing.
Go-live preparations | Conducted detailed planning to enable the experience, inclusive of not only the mobile application, but also the physical hardware considerations, supporting processes, and change management (e.g., how might we set up a “Genius Bar” inside the hospital to support device setup?)
What I’m Proud of:
Built and fostered collaboration among a team working across the US and India, including tech leads, developers, designers and an ethnographic researcher.
Successfully advocated for a human-centered design approach. Conducted research with actual individuals diagnosed with CHF, rather than solely relying on nurse input to design the patient app.
Developed a detailed implementation plan and service blueprint for launching the pilot.
Created a repeatable playbook and resources to support the sales and delivery of similar care-at-home engagements for additional healthcare clients.
